What is the purpose of the Immigration Medical Examination?

The United States Citizenship and Immigration Services (USCIS) requires the Immigration Medical Examination to identify the presence of certain conditions that could result in exclusion of prospective migrants from the United States:

3P4Care Avondale has been designated by USCIS as a
Civil Surgeon’s office to perform Immigration Medical Examinations and to report the results to USCIS on Form I-693.

How many visits are required to complete the Immigration Medical Examination?

Most applicants opt for the two-visit service. However, patients aged below 18 or above 44 may consider the one-visit option (COMMING SOON).

What should I bring to the Immigration Medical Examination?

There is no need to bring your Form I-693. As Preparer of your Form I-693, we will provide you with the form.

If I use the two-visit service, do I just come in to pick up my Form I-693 during
the second visit and run?

No. As Preparer of your Form I-693, we are required by USCIS to ensure that you understand all the information entered into that form. Therefore, the second visit is a formal encounter during which we review the test results and check for inaccurate data in your Form I-693. Furthermore, vaccines may be given to complete immunisation requirements or to achieve up-to-date status.

Do I need just two visits to complete my Form I-693?

Most applicants typically complete their Immigration Medical Examination and Form I-693 within two visits. Rarely, additional visits may be necessary, if more testing, treatment or referral is needed because of abnormal findings.

How much does it cost to complete the immigration medical examination?

The basic fee, in full, discounted or with rebates, covers:

Additional fees apply to screening tests for tuberculosis, syphilis and gonorrhoea, but you can use your insurance to cover those expenses. If the tuberculosis blood testing reveals prior exposure to tuberculosis, a chest x-ray will be needed and you can pay the radiologic facility directly.

Bringing all vaccination records or immunity test results will help you save money.

If vaccinations are not up-to-date, you can opt for

You can obtain vaccinations from your primary care doctor or retail pharmacy and bring the reports to us before your second visit. Alternatively, they are provided AT COST PRICES (plus injection fees) at 3P4Care Avondale.

Further investigations and treatments for any diseases identified are not part of the Immigration Medical Examination. The applicant may choose to have either the Civil Surgeon or his/her primary care doctor provide the follow-up interventions at additional costs.

What is Special Clinic (discount session) for Immigration Medical Examination?

Special Clinics group multiple patients seeking Immigration Medical Examination together in the same session, so we can save time by explaining USCIS requirements.to multiple applicants at the same time. However, none of your personal information will be divulged in the group session. The only information other attendees of the Special Clinic know about you is that you are seeking an Immigration Medical Examination to complete Form I-693. Your clinical examination will be individually conducted in total privacy and confidentiality in a consulting room.

While the basic fee is $250.00, you can potentially bring it down to $200.00 if you attend the Special Clinic (see below) for both your first and second visits:
